Real Solutions strives to remain at the forefront of advances in our field. With this goal in mind we have created numerous care programs with which to better care for individual patient needs.
These programs help us to better educate our patients and caregivers while monitoring certain disease processes and chronic conditions. This is all designed to help us minimize hospital re-admissions and keep our patients comfortably at home.
The expected outcome of our program is to improve functional abilities through symptom and lifestyle management. Our program is aimed at minimizing hospitalization by teaching patients self-management skills and providing them with the necessary tools to regain independence.

Our Programs
CHF Rehospitilization
A new diagnosis or chronic diagnosis of CHF can be daunting to manage on your own. Our nurses and therapists will reinforce with you guidelines to follow to prevent a CHF exacerbation and re-hospitalization.
Cardiac Care
Whether you are going home from the hospital after cardiac bypass surgery, pacemaker/defibrillator implant, a heart attack or you are in good hands with our rehab at home program.
Pulmonary Care
Whether you are dealing with chronic conditions like COPD, Asthma or cancer or acute respiratory infections like pneumonia or COVID-19 our nurses and therapists can assist to improve your quality of life, promote independence, speed up your recovery time and prevent re-hospitalizations.
Orthopedic Surgery Aftercare
Following Orthopedic surgery our therapist can follow your doctor’s protocol and aide you in recovering from surgery to get you ready to go to outpatient therapy.
Wound Care
Wounds can be difficult to manage especially if they are chronic and non-healing. Our nurses are skilled at providing the most up to date wound care protocols and dressings including wound vacs, sterile dressings, managing pressure injuries, diabetic ulcers, amputations and many other types of wounds.
Diabetes Care
If you are new to diabetes or have been diagnoses with it for years we are here to help. Diabetes can change as you age and our nurses are excellent at teaching clients to manage their long term chronic conditions to maintain a healthy lifestyle.
Lymphedema Care
Managing lymphedema, at times, calls for professional assistance. Let our Lymphedema Certified therapists assist you in keeping lymphedema under control with wraps, stockings, compression devices and prescribed medications
Medication Management
Our nurses are very seasoned and skilled with medication and disease management. They can assist with new or changed medications and monitoring for side effects, provide prescribed injections and IV infusions.
